We present a case of a 68-year-old woman who was admitted with Q-wave inferior myocardial infarction. Auscultation revealed a harsh pansystolic murmur all over the precordium. Transthoracic echocardiography visualized ventricular double rupture, consisting of ventricular septal rupture, and rupture of the right ventricular free wall with pseudoaneurysm formation. Surgical correction could not be performed because of the patient’s refusal. The patient survived for 2 years and 7 months.

Key words: ventricular double rupture, interventricular septal rupture, right ventricular free wall rupture, pseudoaneurysm
