Myocardial revascularization in patients with ischemic heart disease and diabetes mellitus

The contemporary data regarding myocardial revascularization in patients with ischemic heart disease and diabetes mellitus are summarized in the state-of-the-art article. Based on evidence-based medicine, coronary artery bypass grafting (CABG) is a most effective revascularization method in patients with ischemic heart disease and concomitant diabetes mellitus. Surgical myocardial revascularization decreased number of hard endpoints in patients with diabetes mellitus and multivessel coronary artery disease, compared to percutaneous coronary intervention (PCI), in the FREEDOM study. In case of PCI it is reasonable to use eluting-stents which decrease probability of restenosis and necessity in repeat revascularization. The influence of glycemia upon frequency of restenosis after PCI and graft patency after CABG remains unclear.
